You’ll encounter the conference room puzzle in The Medium during your quest to find Richard and enter the Day Room, when, on your way to get some bolt cutters, you’ll have to say goodbye to the souls of The Maw’s other victims.
The “other victims” are two souls: one of a man and one of a woman. The conference room puzzle occurs when you search for the man’s name, so that he can finally leave the spirit world.

Explanation of how to send the souls of the other victims on The Medium
During your quest to obtain the bolt cutters in The Medium, you will encounter two trapped souls: one of a man and one of a woman. If you finally want to reach those elusive bolt cutters, you’ll have to free them both.
The order in which you release the souls is up to you, but you must release them both. To do this, you need to find both his skin and his name.
The trapped soul on the right belongs to a woman and you will find everything you need to free her by following the spirit trail to your right. It will involve stealth and minor puzzle solving, but it’s nothing that complex.
The trapped soul on the left belongs to a man and you will easily find his mask by following the spirit trail to your left. However, you may need help figuring out this man’s name.
How to Solve the Conference Room Puzzle on The Medium
Before we begin, it’s important to know that you need to be very careful with the items and what order you interact in during the conference room puzzle in The Medium. Otherwise, you risk losing the ability to interact with certain items and not being able to complete the puzzle.
The only way to fix this situation is to reload one of your previous autosaves and try to solve the puzzle again.
With that in mind, let’s solve the conference room puzzle!
The conference room puzzle is actually divided into two different rooms; The first is accessed by traveling through the mirror, which is unlocked after collecting the man’s mask in the spirit world.
Find the mask and then go through the mirror.
Once you pass through the mirror, Marianne will find herself in an abandoned conference room; Head to the left side of this room and pick up the phone you’ll find near the open door.
Next, you’ll want to listen to the spirit trail on the phone, so Marianne knows she needs to visit the room across the hall to find out the man’s name.
With this knowledge, leave this room, go up the hallway and through the only open door into the small meeting room.
Once inside, read the sign-in sheet on the table to see who was in the room.

This will also allow you to interact with the chairs, but do not touch them at this time. Doing so may prevent you from interacting with specific items, thus blocking the puzzle and your progress in the game.
Instead, you must head to the back of the room and interact with the ashtray.

Listening to the spirit trail at the base of the tray will inform you that the man you are looking for didn’t like the fact that he had been seated across from someone named Boris.
With this knowledge, go back to the attendance sheet, still ignoring those chairs, and Marianne will notice that Boris was sitting in seat number nine.

Once you’ve inspected the chair, walk over to the one in front of you to discover that it’s number four.

Return to the timesheet one last time and Marianne will discover the real name of the soul she wishes to save.
Now you can go back to the trapped soul and see it. Just keep an eye out for the Maw, which will be invisible, and exit through the door of the room containing the mirror as you try to leave.
The easiest way to avoid the Maw here is, while holding Marianne’s breath and crouching, to stay on the left side of the hallway, until the Maw comes out the door and up the hallway. Once you’re out of the door, you can easily enter the room and run to the mirror.
Now all you have to do is free the man’s trapped soul!
