As part of the Search for Cain quest in Act I, this area requires some exploration beforehand. Considering that this mission has many steps to go through, the key is to save as much time as possible.

How to start Cain’s quest in Diablo 2
In Diablo 2, Act I primarily serves as an introduction to both the story and some of the key characters you’ll meet along the way. This includes the notorious Deckard Cain, who becomes trapped on Tristram during the game’s first few hours.
Finding Cain is the third quest in the game and is awarded by Akara in the Rogues’ Camp after completing the Sisters’ Graveyard. It’s quite long and requires you to traverse multiple areas to find a scroll, activate a portal to Tristram, and rescue Deckard Cain.
This quest begins in the Cold Plains, which is the first area of Act I. You will then have to find your way to The Stony Field, which is one step closer to the Monastery Barracks and the Monastery Catacombs.
Unfortunately, the map layouts in Diablo 2 are randomly generated, so we can’t show you the exact location. But don’t worry: the following sections will explain the usual route regardless of the variant, as well as some screenshots that may provide a useful reference.

Diablo 2 Underground Passage Location Explained
Heading from Cold Plains to The Stony Field is pretty easy. The path will divert there, so it won’t be difficult for you to find it.
As usual, be sure to look for the waypoint once you’re on site, to make it easier to backtrack and move through areas later.
Finding the Underground Passage may require some exploration. This is a difficult area from the start, so expect to encounter multiple enemy groups awaiting your arrival.
Fortunately, there is a key reference point that can be helpful. The Underground Passage in Diablo 2 is always located at the edges of the map, following the walls.
See those stones on the walls? Stick to them to find the Underground Passage.
This exponentially reduces the search. Later on there are areas that require you to look for the entrance at different points on the map, but that’s not the case here.
While it’s not possible to guess where exactly the entrance will appear, the safest bet is to stick to the walls until you finally come across it.
Once you’re ready, go ahead and enter. The Underground Passage has two levels: you can reach the Dark Forest from the first to continue the Search for Cain quest, or continue exploring the floor below in search of loot. In any case, you can come back here later.
