What’s inside Patches’ chest?
Sparing Patches will unlock you as a vendor, which gives you a number of useful items to help you early in the game.

Where to find patches after Murkwater Cave
Patches will not remain in Murkwater Caves and will, in fact, appear elsewhere once you progress beyond Castle Stormveil.

When you have completed your first kill, talk to Patches again and he will give you a card with his own kill target.
The target is Great Horned Tragoth, who you may have seen as an NPC you can summon during the fight against Starscourge Radhan.
He can be found on the Ruin-Covered Precipice, which should be familiar to players if they decide to find an alternative path to the Altus Plateau instead of finding the medallion halves to operate the Great Dectus Elevator.
In short, this path is located at the end of the ravine north of Liurnia. Tragoth’s Invading Sign can be found in the same place where he fights Magma Wyrm Makar.

Where to find patches after Volcano Manor
To continue your search for Patches and get him out of Volcano Manor, you’ll need to continue the mansion’s quest line, which has you fighting and defeating the God-Eating Serpent (also known as Lord Rykard).
The next place you will find Patches is the Shadowed Castle, which you will have visited if you follow Millicent’s quest line.

Talk to him and he will give you the Dancer’s Castanets item and tell you to give it to Lady Tanith before apparently dying.
At this point, Tanith and her knight will have left the mansion. However, if you fast travel back to the Siege of Grace where you fought Rykard, you will find her lying next to her fallen lord, eating him.
You can talk to her and have the option to give her the Dancer’s Castanets although she will ignore you. In any case, doing this will make her key items disappear, but it doesn’t seem to do anything else.
You can now leave the area or kill Tanith if you want to trigger an optional fight.
Where to find patches after Shaded Castle
April patch 1.04 added a final part of the Patches quest. So, having given the dancer’s castanets to Tanith, he returns to the Murkwater caves. If you head to where Patches was originally, you will find that the area has a wall of fog once again.
Enter and you will find another chest, which only has a glass shard inside. It’s deja vu, as once again Patches will ambush you from above with his spear and his large shield.
You won’t even have to fight him, though, as he’ll suddenly recognize you and surrender almost immediately. You’ll automatically be rewarded with Patches’ Crouch emote, which will allow you to crouch like he does.
That’s basically the end of Patches’ questline and he’ll stay in Murkwater Caves to sell the same things as before.
Or you can also kill him to take his equipment (a set of leather armor and a +7 spear, so nothing to write home about) and Patches’ Bell Bearing, which can be given to Twin Maiden Husks in the Roundtable Hold. , although instead it will simply be buying them the same things.
